More Options / Level Select
Go to the Options screen and move the pointer Down to "Exit."  Hold Down 
and press A, B, B, A, B, A, A, B, A, A, B, A, A.  There will be a screen from 
which you can get infinite lives, continues, weapons, etc., as well as choose 
a Starting level.
